Make warnings stackable

This also does not mean that you are "free to break the rules again". If a user does not adhere to the warning given to them their account will receive a higher punishment likely receive an appropriate ban.

I believe this warning was placed after the offense so Barry didn't warn him again because really what is the sense of giving two warnings. If the user was continuing the offense after the warning was placed then a further punishment is due because the user is clearly not getting the message.
